Nine (9) Items to have ready for your appointment:
1) You must complete the online portion of your FAA exam by registering or logging into Med Xpress. This is done with all new applicants, as well as with every renewal. Once you have completed all of the questions regarding your past medical history and symptoms, the program will generate a CONFIRMATION number for you. It is imperative that you bring this confirmation number with you to the appointment. You may use this link to access MedXPress
2) Valid photo ID
3) Face mask is optional, but will be required if you are recovering from any Cold/viral symptoms. If you are actively having infectious symptoms, please call to reschedule your appointment.
4) Do not urinate prior to the exam as a small urine sample is needed for testing. I advise drinking only water to hydrate, avoiding caffeinated drinks as it may negatively influence your vital signs.
5) If you have any of the following medical conditions, you will need to provide a letter(s) from your treating medical provider regarding the status of your condition: (from the FAA website)
6) *Payment accepted: check, cash, and Zelle. Credit cards are NOT accepted.
7) Updated eyeglasses or contact lenses, if needed.
8) US Military Veterans: a copy of your most up-to-date VA Benefits Ratings Decision.
9) If you have been previously issued any FAA Special Issuances (aka SIs or AASIs), or have been CACI-qualified by the FAA, please send the paperwork to me approximately 2 weeks ahead of your scheduled appointment. Please send to: [email protected].
